Over the recent years ALL has been focusing on delivering individualised care and patient-centred re-engineering of health care institutions. It has developed complex models, test methods and inference techniques for refining diagnostic descriptions and selecting customised optimal treatment. ALL has also developed new models of patient-centred innovative health care organisations.
ALL has developed a methodology for identifying and simulating large complex models synthesising knowledge at different levels. Special applications covered intelligent information and partner systems including algorithm-based patient management systems, holistic diagnostic and treatment planning modules.
Chronic care model
Although the importance of chronic care has become widely recognised, little has been done to change the way medical care is organised and practised to respond to this challenge. ALL has developed special risk driven integrated chronic care model which supports personalised, proactive, preventive and participatory care.
The proposed model provides telemedical solutions to support the care of patients with chronic diseases. Among others, it introduces televisits (where the patient and the healthcare provider communicate via telephone or email) and virtual visits (where the patient communicates with intelligent computer systems) in order to decrease the number of contact visits. The most adequate visit type is determined automatically based on the patient’s needs. These various visit types put patients in a complex health care and social network in which continuous supervision and support are available whenever needed. The different services are underpinned by an intelligent information system which supports evidence based medicine, and provides pro-active care for patients based on risk management.
ALL proposes an info-communication platform that integrates a new personalized service model for the management of widespread chronic diseases while it also considers individual expectations of the patients concerning life quality. The unified information space supports the processes of model- and knowledge-based decision making. It supports the learning process and the development of every participant.
The proposed model also provides effective treatment planning and disease management. It makes it possible to form closed loops in the cyclic process of the treatment. It is an important characteristic of the model that activation and operation of different loops is determined by the status of the patient.

ALL has elaborated descriptive and analytical methods and tools for non-linear systems and tools for logical modelling and also their application within the field of biomedicine. ALL is also active in the modelling of care processes. ALL has developed special approach on the basis of systems biomedicine to personalisation of the care.
